The main criteria of review of form HE39 and form HE40 are:

• company name and number;
• sworn signatures of director and secretary of the company (in case where the officer is a company, the company seal is required);
• signature on each list attached.

Most common reasons for rejection of the form by the examiner:

• the form submitted is not the statutory form;
• failure to complete/ submit the above details.
